CC: smelling vagina discharge and burning urination
HPI: MF is the 21-year-old GO P0 L0 Hispanic female who presents to the clinic with a chief complaint of smelly vagina discharge and burning with urination for six days. She reports having unprotected with her recent boyfriend. She reports occasional cervical pain during sexual intercourse. She rates the dysuria 6/10 and reports taking some Tylenol with minimal relief. She denies fever, chills, or abdominal pain. She reports that her boyfriend also has burning urination.
Current Medications:
Tylenol 650 mg PO as needed for pain Sprintec .25mg/35mcg PO daily Allergies: No known drug allergies
Past Medical History: UTI 12 months ago. Treated with antibiotic.
OB History: Age of Menarche 10. 28 days regular Menstrual cycle .Last Menstrual Period : 01/06/2022. Menses last five days. Regular flow.
Has not had a pap smear.

Social History: MF is a 4th year engineering college student. She lives in an apartment near her college with two roommates. She denies tobacco use, drinks alcohol occasionally when out with her friends. She denies illicit drug use.
